Central venous pressure (CVP) has traditionally been used as a marker of intravascular volume and cardiac preload. However, recent evidence suggests that its utility as a fixed target for fluid resuscitation is diminishing. This narrative review traces the historical context of CVP interpretation and emphasizes the need for a multimodal approach to hemodynamic assessment, integrating CVP with other techniques such as venous excess ultrasound (VExUS) to better evaluate patient fluid status and organ perfusion.
