Hepatic encephalopathy (HE) is a serious neuropsychiatric complication of cirrhosis that can lead to significant morbidity and hospital readmissions. The 2026 American College of Gastroenterology (ACG) guideline presents 24 recommendations addressing the management of both covert and overt HE, including diagnostic evaluation, treatment options, and strategies to prevent readmission. Key recommendations include the use of lactulose and rifaximin for treatment, nutritional advice regarding protein intake, and counseling for patients regarding driving after episodes of HE. This guideline aims to improve patient outcomes through evidence-based practices.
