Simultaneous heart–kidney transplantation (SHKT) has become increasingly common, with evolving recipient characteristics and outcomes influenced by the 2018 UNOS heart allocation policy change. This study utilizes a UNOS registry analysis to evaluate immunosuppression strategies among adult SHKT recipients from January 2013 to June 2025. The findings aim to fill knowledge gaps regarding immunosuppression practices and their impact on infection and mortality rates in this patient population.
