Portal vein thromboses (PVTs) are common in patients with cirrhosis and are associated with advanced portal hypertension and mortality. The treatment of PVTs remains a clinical challenge due to limited evidence and competing risks of PVT-associated complications versus the bleeding risk of anticoagulation. This expert review, commissioned by the American Gastroenterological Association (AGA), aims to provide timely guidance on the management of PVT, addressing the significant heterogeneity in PVT phenotype and the emerging spectrum of therapeutic options.
