Coronary artery calcium (CAC) scoring has become a crucial marker for assessing subclinical atherosclerosis and enhancing cardiovascular risk stratification. The recommendations regarding CAC have evolved significantly, transitioning from a Class IIb recommendation in 2007 to a Class I recommendation in the 2026 guidelines for specific populations. This report reviews these changes and highlights the expanding role of CAC in personalized cardiovascular prevention strategies, including its implications for treatment intensity and LDL-C goals.
