Malignant mitral valve prolapse (MVP) presents a heterogeneous clinical spectrum, with some patients facing a benign course while others are at risk for sudden cardiac death (SCD). The review highlights key electrocardiographic markers and morphofunctional characteristics that can help in identifying high-risk patients. It emphasizes the importance of early identification of complex arrhythmias that may occur before macrofibrosis is detectable. The findings suggest that newer imaging techniques may provide additional insights into the risk stratification of patients with MVP.
