Most patients with severe DMR may eventually require surgery, but the timing for intervention in asymptomatic patients remains controversial. Current guidelines are based on nonrandomized studies and expert opinion, leading to a need for a comprehensive approach that includes serial echocardiography and various imaging modalities for risk stratification. The review highlights the importance of understanding surgical options and outcomes, as well as the necessity for close follow-up in these patients.
