Atherosclerotic cardiovascular disease (ASCVD) remains the leading cause of mortality, despite advancements in lipid-lowering therapies. Fibrates have historically been used to manage dyslipidaemia, but their use has declined due to neutral outcomes in large trials. This document explores the potential continued relevance of fibrates, especially for specific patient subgroups facing therapeutic challenges, such as those with insulin resistance or severe hypertriglyceridaemia. The review also considers the expanding potential of fibrates in managing metabolic dysfunction-associated conditions.
