The ductus arteriosus is crucial for fetal circulation but often remains patent in extremely preterm infants, leading to various complications. Despite decades of research, the optimal treatment for PDA in this vulnerable population remains unclear, with significant variability in clinical practice. Recent findings from the BeNeDuctus trial suggest that early treatment may do more harm than good, prompting a reconsideration of management strategies in favor of expectant approaches.
