Cortical lesions have transitioned from being largely overlooked to a central aspect of multiple sclerosis pathophysiology. This review traces the historical understanding of cortical demyelination, focusing on the relationship between cortical pathology and clinical outcomes. It examines the role of advanced imaging techniques in detecting and monitoring these lesions, while also addressing the limitations of current methodologies and the potential for future research to refine therapeutic strategies.
