Significant unprotected left main coronary artery (ULMCA) disease is encountered in approximately 5% of patients undergoing diagnostic coronary angiography. IVUS overcomes many limitations of angiography and improves outcomes in patients undergoing PCI. The review includes a comprehensive search of studies assessing the effects of IVUS-guided versus angiography-guided ULMCA PCI on various endpoints, highlighting differences in event rates over time.
